What is Coordinated Community Care (CCC)?

CCC is a Medi-Cal managed care benefit that addresses clinical and non-clinical needs of individuals through the coordination of services and comprehensive care management. Medi-Cal partners with providers, like the Good News Rescue Mission, to provide members with long-term support, coordinating services across various platforms to help address your needs. 

How will this benefit you?

Your assigned case coordinator will be available to offer you the support you need and listen to what those needs are. They provide you with:

  • Long-term coordination, which means you have someone you can build trust with who will be familiar with your needs.
  • Support to help reduce barriers that made it difficult for you to access services.
  • An extra level of help that goes beyond what is provided to you at a traditional hospital.
  • Easier access to in-person care by conveniently meeting you where you live.
  • Transportation to pick-up medication and take you to your appointments. 

Who is eligible for CCC?

  • Adults, unaccompanied youth and children, and families experiencing homelessness.
  • Those who are at risk for avoidable hospital or emergency department care.
  • Those with serious mental health and/or substance use disorder needs.
  • Adults living in the community and at risk for long-term care institutionalization.
  • Adult nursing facility residents transitioning to the community.
  • Children and youth enrolled in California Children’s Services (CCS) or CCS Whole Child Model with additional needs beyond their CCS condition(s).
  • Children and youth involved in child welfare (foster care).
  • Adults and youth who are transitioning from incarceration.
