What is Community Supports?
Community Supports are services that help improve Medi-Cal Managed Care Plan (MCP) members’ overall health by addressing needs like housing, meals, and personal care. They are designed to provide a range of services and resources to address the unique needs of Medi-Cal members in the community, with a focus on promoting health, stability, and independence.
The Good News Rescue Mission is an authorized Medi-Cal Managed Care Plan provider who will assign you a care coordinator to help you access services across various platforms that help address your needs.

- Short-Term Post-Hospitalization Housing
- Recuperative Care (Medical Respite)
- Day Habilitation Programs
- Caregiver Respite Services
- Personal Care and Homemaker Services
- Nursing Facility Transition/Diversion to Assisted Living Facilities
- Community Transition Services/Nursing Facility Transition to a Home
- Environmental Accessibility Adaptations (Home Modifications)
- Medically Tailored Meals/Medically-Supportive Food
- Sobering Centers
- Asthma Remediation

Who is elibible?
These services are available to eligible Medi-Cal members regardless of whether you qualify for CCC services. All Medi-Cal managed care plans will offer as many of the pre-approved Community Supports services as possible if available in your area.
